**VOLUNTEER Paperwork Required**

It is very important to obtain volunteer clearance at the school site as early as possible.  More information on volunteer requirements can be found on the district website here.

The following 3 forms must be completed and submitted to the school office.

1. Submit district school volunteer application
2. Category C Background Check 
3. Obtain TB clearance  (Stacey Hoffmayer can help with TB clearance. Email president@fodacms.org for help.) 

The best way to stay involved in your child's school is by volunteering. We need help in a variety of areas and the Friends of Dana and Correia Middle Schools has many wonderful opportunities to support your child as well as the Middle Schools they attend.
